What is Inflammatory Bowel Disease (IBD)?
Inflammatory Bowel Disease (IBD) encompasses two major chronic inflammatory conditions of the gastrointestinal tract, Crohn's Disease (which can affect any part of the GI tract, from mouth to anus) and Ulcerative Colitis (limited to the colon and rectum). Both cause periods of active inflammation (flares) and remission. Crohn's Disease is characterised by transmural inflammation and can cause strictures, fistulas, and abscesses. Treatment Approach
Surgical IBD Care, Bowel-Preserving, Complication-Focused
IBD treatment follows the principle of bowel preservation. Surgery is reserved for complications of Crohn's disease, strictures (treated with ileocaecal resection or stricturoplasty), abscesses, fistulas, and medically refractory disease. Segmental resection removes only the diseased bowel segment. Perianal Crohn's disease is managed with combined medical and surgical expertise.

Medical Management First
Aminosalicylates, steroids, immunomodulators (azathioprine, methotrexate), and biologics (infliximab, adalimumab, vedolizumab) are the backbone of IBD management. Surgery is considered for complications and refractory disease.

Ileocaecal Resection (Crohn's, Terminal Ileum)
The most common Crohn's surgery, removal of the diseased terminal ileum and caecum with ileocolic anastomosis. Bowel-sparing technique, laparoscopic approach preferred.

Stricturoplasty
Widening of a Crohn's stricture without bowel resection, preserves intestinal length and absorptive capacity. Multiple stricturoplasties may be performed simultaneously.

Segmental Bowel Resection
Removal of a specific diseased bowel segment for localised Crohn's disease causing obstruction, abscess, or fistula not responding to medical therapy. Bowel continuity is restored where possible.
