EUA + Drainage (Examination Under Anaesthesia)
Complete assessment and thorough drainage of deep, complex, or incompletely drained perianal abscesses under general anaesthesia - ensuring no residual pus and early identification of underlying fistula tracts.
What is EUA + Drainage (Examination Under Anaesthesia)?
EUA + Drainage is used for perianal abscesses that are deep, large, complex, incompletely drained by a prior procedure, or where full clinical assessment is not possible in the clinic. Under anaesthesia, the surgeon performs a thorough bimanual examination, identifies all abscess loculations, and ensures complete drainage. Dr. Samhitha Reddy performs EUA + Drainage at Lux Hospitals, Hyderabad for ischiorectal, intersphincteric, and horseshoe abscesses where thorough drainage under controlled conditions is essential. As part of the perianal abscess treatment in Hyderabad programme, EUA is also the opportunity to identify early fistula tracts - providing a combined diagnostic and therapeutic procedure under a single anaesthetic.
How the Procedure Works
Assessment & Imaging
Perianal ultrasound or MRI is used pre-operatively for deep or horseshoe abscesses to map the extent of the collection and identify relevant anatomy.
General or Spinal Anaesthesia
Full anaesthesia allows complete relaxation - essential for thorough bimanual assessment of deep ischiorectal or intersphincteric abscesses.
Bimanual Examination
A systematic bimanual examination identifies all abscess compartments, any fistula tract, and the internal opening. Secondary collections in adjacent spaces are mapped.
Complete Drainage
The abscess is incised and all loculations are broken down. Thorough saline washout. Counter-drainage for large bilateral (horseshoe) abscesses if needed.
Seton (if Fistula Identified)
If a clear internal opening and fistula tract are identified, a loose draining seton may be placed at the same sitting. Definitive fistula management is then planned.
Outcomes
Who Needs This Treatment?
- →Allows complete, thorough examination not possible in an awake patient
- →Ensures all abscess loculations are identified and drained - no residual collection
- →Early identification of fistula tract reduces subsequent diagnostic delay
- →Combined diagnostic and therapeutic procedure under a single anaesthetic
- →Prevents recurrence from incompletely drained abscesses
- →Available as a same-day or next-day emergency procedure at Lux Hospitals, Hyderabad
EUA is an invaluable tool for complex abscesses. Under full anaesthesia, we examine the entire anorectal space methodically - finding collections that were not apparent on clinic examination and mapping fistula tracts that will determine the patient's subsequent treatment journey.
