What is Haemorrhoidectomy (Milligan-Morgan / Ferguson)?

Haemorrhoidectomy is the surgical excision of haemorrhoidal tissue and remains the most definitive treatment for severe piles. The Milligan-Morgan technique leaves the excision wounds open to heal by secondary intention, while the Ferguson technique uses absorbable sutures for primary wound closure - offering faster healing in selected cases. Dr. Samhitha Reddy performs haemorrhoidectomy at Lux Hospitals, Hyderabad for Grade III–IV haemorrhoids, large external disease, or cases where minimally invasive procedures are not sufficient. As one of the most experienced piles doctors in Hyderabad, Dr. Samhitha uses meticulous technique to achieve complete disease resolution with minimal post-operative discomfort and complications.

Indicated for Grade III–IV haemorrhoids, large external haemorrhoids, or when minimally invasive procedures are contraindicated or have previously failed. Dr. Samhitha will discuss all options at consultation.

How the Procedure Works

1

Pre-operative Assessment

Blood tests, ECG, and anaesthesia assessment. Enema bowel preparation on the morning of surgery.

2

Anaesthesia

Spinal or general anaesthesia. The patient is positioned in the lithotomy or left lateral position for optimal surgical access.

3

Haemorrhoidal Excision

Each haemorrhoidal bundle (typically 3 positions - 3, 7, and 11 o'clock) is individually clamped at its pedicle, excised, and ligated securely.

4

Wound Management

Milligan-Morgan: wounds left open for secondary healing. Ferguson: wounds closed with absorbable sutures. Adequate skin bridges preserved to prevent anal stenosis.

5

Post-operative Care

Hospital stay 1–2 days. Regular sitz baths, stool softeners, appropriate analgesia, and high-fibre diet. Full recovery in 2–3 weeks.
